David James Perkins

SOUTH BEND — David James Perkins; 35, of Chicago, formerly of South Bend, died at 10:30 p.m. Sunday in Northwestern Memorial Hospital, Chicago, after an illness. Mr. Perkins was a waiter at Hyatt Regency Hotel, Chicago, and was co-artistic director for Blind Parrot Productions, Chicago. He was born Aug. 26, 1955, in South Bend and lived in Chicago since 1977, moving from Bloomington, IN.

He is survived by his mother, Annabelle Perkins of South Bend; and a brother, Daniel of Mishawaka.

He was a graduate of Indiana University, Bloomington.

Services were to be at 1 p.m. today at Hollis-Orvis-Palmer Funeral Home, 2528 Mishawaka Ave. Burial was to be at Chapel Hill Memorial Gardens, Osceola. Friends may call two hours before services in the funeral home. Memorial contributions may be made to Coalbush United Methodist Church, Mishawaka.
