SOUTH BEND - David G. Fields, 50, of New Albany, Ind., formerly of South Bend, died Thursday in Audubon Regional Medical Center, Louisville, Ky.
Mr. Fields was employed by B & J Trucking Co., Borden, Ind., as a truck driver. He was born March 1, 1945, in South Bend.
He is survived by his wife, Susan (Hertel) Fields of New Albany; three daughters, Tammy E. Fields, Diana E. Fields and Debbie E. Fields, all of New Albany; two sons, Jim and Ted, both of Machesney Park, Ill.; his father, Stanley L. of Summerfield, Fla.; and a sister, Sherrie Fields of Ocala, Fla.
Services will be at 10:30 a.m. Monday in St. Raphael Catholic Church, Louisville. Burial will be in Calvary Cemetery, Louisville.
Friends may call from 2 to 9 p.m. Saturday and Sunday in Ratterman Funeral Home, Louisville.
Memorial contributions may be made to the American Heart Association.
